Introduction

The 12-week Down Syndrome Screening Scan is a key part of early pregnancy care. Performed between 11+2 and 14+1 weeks, it helps assess the likelihood of chromosomal conditions such as Down syndrome (Trisomy 21), Edwards syndrome (Trisomy 18), and Patau syndrome (Trisomy 13).

At Holistic Gynaecology Clinic London, we proudly offer this important scan using our brand-new Sonoscape P60 ultrasound system with AI-enhanced fetal imaging, together with the expertise of Dr. Nele Dumpert, Consultant Community Gynaecologist.

What Is the 12-Week Down Syndrome Scan?

This ultrasound examines early fetal development and measures specific biometric markers that are essential for calculating the risk of chromosomal conditions.
The scan is usually combined with a maternal blood test to create a personalised risk assessment.

The Four Main Ultrasound Measurements Required for Laboratory Risk Calculation

To generate an accurate screening result, laboratories typically require the following four key fetal biometrics:

1. Nuchal Translucency (NT)

Measurement of the fluid behind the baby’s neck.

  • A crucial marker in first-trimester screening
  • Helps detect increased risk for chromosomal abnormalities

2. Crown–Rump Length (CRL)

Measures the baby’s length from head to bottom.

  • Confirms gestational age
  • Ensures eligibility for first-trimester screening (11–14 weeks)

3. Head Circumference (HC)

Measures the size of the baby’s head.

  • Assesses early fetal growth
  • Supports accuracy when analysed together with other biometrics

4. Biparietal Diameter (BPD)

Measures the width of the fetal head.

  • Supports consistent biometric evaluation
  • Helps confirm the stage of pregnancy and fetal development

The Blood Test Component

Alongside the ultrasound, the laboratory also requires two maternal blood markers:

  • Free β-hCG
  • PAPP-A (Pregnancy-Associated Plasma Protein A)

The blood test is combined with the four ultrasound measurements to create a personalised risk calculation for Down syndrome and other trisomies.

What Happens After the Scan?

After your ultrasound, you will receive:

  • Your report including NT, CRL, HC, and BPD
  • Images of your baby
  • Information for completing your blood test (if not yet done)

Once the laboratory analyses both the scan and blood results, you receive a personalised risk estimate.

A high-risk result does not confirm a diagnosis—it simply indicates the need for further testing such as NIPT, CVS, or amniocentesis.

Is This the Same as NIPT?

No. The 12-week scan is part of the Combined Screening Pathway, whereas NIPT uses fetal DNA from the mother’s blood to assess chromosome risks with higher sensitivity.
Many families choose to do both for added reassurance.
