Prescribing Policy

Patients registered at Holistic Gynaecology Clinic London, will receive medication based on clinical necessity.

Upon registration, new patients will be queried regarding their current medication on the New Patient Registration Form and during their New Patient Review Appointment. A prescriber will then incorporate this information into their record.

The Practice adheres to a policy of prescribing medication within licensed parameters.

Details of prescribed medication are promptly recorded in the patient record.

In instances where hospitals alter a patient’s medication regimen (additions, discontinuations, or modifications), the Practice ensures timely incorporation of this information into the patient record. Relevant documents are scanned into the patient record, and the patient’s doctor is notified to update the record accordingly and issue any necessary prescriptions.

When the maximum number of authorized repeat prescriptions has been reached, the prescriber is alerted to reauthorize prescriptions, and this action is documented in the patient record.

It is the Practice’s policy to conduct medication reviews for all patients at least annually.

Reported allergies or sensitivities are documented in the patient record.

Upon prescribing medication, the prescriber informs the patient about potential side effects and advises them to seek guidance if such effects occur. The patient record is annotated to confirm the provision of this advice.

All prescribed medication is associated with a diagnosis in the patient record and appropriately coded using Read Codes, where feasible within the clinical system.

Patients receive guidance on medication administration and timing, which is also printed on both the prescription and dispensing label.
