Holistic Menopause Care in London: A Personalised Approach to Menopause Health

Menopause is a natural stage of a woman’s life, but the experience is different for every woman. For some, the transition happens relatively smoothly. For others, changing hormone levels can affect sleep, mood, energy, weight, sexual health, concentration, joints and overall quality of life.

At Holistic Menopause Care London, we recognise that every woman’s menopause journey is unique and deserves a personalised approach that considers her symptoms, hormonal health, lifestyle and individual needs.

At Holistic Gynaecology Clinic London, we believe menopause care should look at the whole woman rather than individual symptoms in isolation.

Our approach to holistic menopause care in London combines evidence-based gynaecological medicine with careful consideration of hormonal health, nutrition, lifestyle, sleep, stress, exercise and emotional wellbeing.

Led by Dr Nele Dumpert, an experienced gynaecologist with a special interest and expertise in women’s hormonal health, perimenopause and menopause, our clinic provides personalised menopause consultations in London as well as online menopause consultations for women across the UK.

What Is Menopause? Understanding Your Menopause Journey

Menopause is defined as the point when a woman has not had a menstrual period for 12 consecutive months, provided there is no other medical reason for her periods to have stopped.

However, hormonal changes usually begin well before the final menstrual period.

This transitional stage is known as perimenopause.

During perimenopause, ovarian hormone production becomes more variable. Oestrogen levels can fluctuate considerably and ovulation may become less regular, which can also affect progesterone production.

This is one reason menopause symptoms do not always appear in a simple or predictable pattern.

A woman may experience symptoms while she is still having regular or relatively regular periods.

According to current NICE guidance, in otherwise healthy people aged 45 or over who have typical menopause-associated symptoms, perimenopause and menopause can usually be identified clinically without routine laboratory testing.

Common Perimenopause and Menopause Symptoms

Menopause involves much more than hot flushes.

Hormonal changes during the menopause transition can affect many areas of physical and emotional health. Symptoms can vary considerably between women and may change as the transition progresses.

Common perimenopause and menopause symptoms include:

  • Hot flushes
  • Night sweats
  • Changes in menstrual cycles
  • Heavy or irregular periods during perimenopause
  • Difficulty sleeping or waking during the night
  • Fatigue
  • Brain fog
  • Difficulty concentrating
  • Memory changes
  • Anxiety
  • Low mood
  • Irritability
  • Reduced resilience to stress
  • Heart palpitations
  • Headaches or migraines
  • Joint and muscle pain
  • Changes in body composition
  • Menopause weight gain, particularly around the abdomen
  • Reduced muscle mass
  • Vaginal dryness
  • Vulval discomfort
  • Pain during intercourse
  • Urinary symptoms
  • Recurrent urinary tract infections
  • Reduced libido
  • Breast tenderness
  • Changes in skin and hair

NICE recognises that menopause-associated symptoms may include vasomotor symptoms such as hot flushes and sweating, mood changes, musculoskeletal symptoms, genitourinary symptoms and sexual difficulties.

For this reason, good menopause care should consider the overall pattern of symptoms rather than treating each complaint as though it were unrelated.

What Does Holistic Menopause Care Mean?

The word holistic does not mean rejecting conventional medicine.

For us, holistic menopause care means combining appropriate medical treatment with a broader understanding of the factors that influence a woman’s hormonal and general health.

The British Menopause Society supports a holistic and individualised approach to menopause assessment, including lifestyle and dietary advice alongside discussion of HRT where appropriate.

At Holistic Gynaecology Clinic London, we therefore look beyond the question:

“Do you need HRT?”

Instead, we ask:

“What is happening in your body, what symptoms are affecting you, what are your individual health priorities, and what combination of treatments could best support you?”

Our Holistic Menopause Assessment in London

A detailed menopause consultation gives us time to understand your individual experience.

Depending on your circumstances, Dr Nele may explore:

Your Menstrual and Hormonal Health During Menopause

Understanding how your periods have changed can provide important information about your stage of the menopause transition.

We may discuss changes in cycle length, bleeding, premenstrual symptoms and when your symptoms occur in relation to your menstrual cycle.

Your Menopause Symptoms and Hormonal Changes

Rather than concentrating on one symptom, we consider your overall pattern of symptoms and which ones are having the greatest impact on your daily life.

Sleep Problems During Perimenopause and Menopause

Poor sleep can have a substantial effect on mood, appetite, concentration, energy and quality of life.

Night sweats may disrupt sleep, but menopause-related sleep problems can occur even without obvious night sweats.

Menopause Nutrition and Metabolic Health

Nutrition becomes particularly important during midlife.

We may discuss protein intake, blood sugar stability, fibre, healthy fats and micronutrient intake as part of a broader strategy to support metabolic, cardiovascular and bone health.

Menopause Weight Gain and Body Composition

Many women notice that maintaining their previous weight becomes more difficult during perimenopause and menopause.

However, the solution is not necessarily simply to eat less.

Ageing, changing hormone levels, sleep disruption, stress, reduced muscle mass, physical activity and metabolic factors can all influence body composition.

Our approach focuses on sustainable nutrition, maintaining or increasing muscle mass, appropriate physical activity and identifying other factors that may be contributing to weight changes.

Exercise, Muscle and Bone Health During Menopause

Strength and resistance exercise become increasingly important as women move through menopause.

Maintaining muscle supports physical function, metabolic health and healthy ageing.

Exercise can also support cardiovascular health, bone health, sleep and emotional wellbeing.

Stress, Anxiety and Emotional Wellbeing During Menopause

Perimenopause can sometimes coincide with significant demands from work, family and caring responsibilities.

At the same time, hormonal fluctuations may change how some women experience stress.

Anxiety, irritability, reduced stress tolerance and low mood can therefore be important parts of the menopause picture.

Where appropriate, psychological support or menopause-specific cognitive behavioural therapy may also form part of menopause management. NICE recommends considering menopause-specific CBT for symptoms including vasomotor symptoms, sleep difficulties and depressive symptoms associated with menopause.

HRT and Holistic Menopause Care in London

For many women, Hormone Replacement Therapy (HRT) can be an effective part of menopause treatment.

HRT replaces hormones that decline during menopause, particularly oestrogen.

Oestrogen can be provided in several forms, including:

  • Transdermal patches
  • Oestrogen gel
  • Oestrogen spray
  • Oral preparations

If a woman has a uterus, adequate progestogen is normally required alongside systemic oestrogen to protect the endometrium.

The appropriate type, dose and route of HRT should be individualised.

NICE recommends discussing the benefits and risks of different approaches, including combined versus oestrogen-only HRT, oral versus transdermal HRT, different oestrogens and progestogens, and sequential versus continuous regimens.

At Holistic Gynaecology Clinic London, HRT is therefore not approached as a standard prescription that is identical for every patient.

Dr Nele considers your symptoms, medical history, menstrual status, personal preferences and individual risk factors when discussing whether HRT may be appropriate.

Body-Identical Hormones for Menopause

Where clinically appropriate, menopause treatment may include regulated body-identical hormones.

The term body-identical refers to hormones with the same molecular structure as hormones naturally produced by the human body, such as estradiol and micronised progesterone.

It is important to distinguish regulated, licensed body-identical HRT from unregulated compounded bioidentical hormone preparations.

The objective is not simply to prescribe hormones, but to find the lowest effective dose and appropriate treatment regimen for the individual woman, with suitable clinical review.

Vaginal Dryness and Genitourinary Symptoms of Menopause

Declining oestrogen can affect the vagina, vulva, bladder and urinary tract.

This collection of symptoms is often referred to as the genitourinary syndrome of menopause (GSM).

Symptoms may include:

  • Vaginal dryness
  • Burning or irritation
  • Pain during intercourse
  • Vulval discomfort
  • Urinary urgency
  • Recurrent urinary infections
  • Changes in sexual comfort

These symptoms are common but should not simply be accepted as an unavoidable consequence of getting older.

Treatment may include vaginal moisturisers and lubricants, as well as local vaginal oestrogen when clinically appropriate.

NHS guidance confirms that oestrogen can be used locally as a cream or pessary to help treat vaginal dryness, soreness and urinary symptoms after menopause.

Menopause, Libido and Sexual Health

Changes in sexual desire during menopause can have multiple causes.

Hormonal changes may contribute, but vaginal discomfort, sleep deprivation, relationship factors, stress, body image and psychological wellbeing may also play a role.

For women experiencing persistent low sexual desire despite appropriate HRT, testosterone may sometimes be considered following individual clinical assessment. NICE recommends considering testosterone supplementation for low sexual desire associated with menopause when HRT alone has not been effective.

Again, treatment should focus on the individual woman rather than a single laboratory result.

Do I Need Hormone Blood Tests Duringr Menopause?

Not necessarily.

This is an important area where more testing does not automatically mean better medicine.

For otherwise healthy women aged 45 or over with typical symptoms, NICE advises that perimenopause and menopause can generally be identified from symptoms and menstrual history without laboratory tests.

Hormone levels such as FSH and oestradiol can fluctuate significantly during perimenopause, so a single blood result may provide a misleading snapshot.

However, blood tests can sometimes be useful when investigating other possible causes of symptoms or when a woman's age or clinical circumstances make additional investigation appropriate.

The question should therefore not be:

“Can we test more hormones?”

It should be:

“Will this test provide useful information that could change our clinical management?”

Menopause Nutrition and Supplements

There is no single supplement programme suitable for every menopausal woman.

Nutritional requirements should ideally be considered in the context of diet, symptoms, medications, medical history and, where clinically appropriate, investigations.

Depending on individual circumstances, areas that may deserve attention include:

  • Adequate protein
  • Calcium
  • Vitamin D
  • Magnesium
  • Omega-3 fatty acids
  • Iron where deficiency is present
  • Vitamin B12 and folate
  • Other micronutrients where deficiency or increased need is suspected

Supplements should complement rather than replace a nutritious diet.

Herbal products also need to be treated with caution. “Natural” does not automatically mean safe, and herbal preparations can interact with prescription medicines.

NHS guidance notes that evidence for some complementary remedies marketed for menopause is limited and that certain preparations can cause significant interactions with medications.

Holistic Menopause Care for Long-Term Health

Menopause care should not focus exclusively on reducing today's symptoms.

It is also an opportunity to think about healthy ageing.

As women move through menopause, it becomes increasingly important to consider:

  • Bone health
  • Cardiovascular health
  • Blood pressure
  • Cholesterol
  • Metabolic health
  • Muscle mass and strength
  • Physical activity
  • Sleep
  • Nutrition
  • Alcohol and smoking
  • Emotional wellbeing
  • Sexual and vaginal health

A holistic menopause consultation provides an opportunity to bring these different aspects of health together.

Their treatment plans should not automatically be the same.

This is the foundation of our approach at Holistic Gynaecology Clinic London.

We combine evidence-based gynaecology with nutrition, lifestyle medicine and personalised hormonal care to help women navigate menopause with greater understanding and confidence.

Our aim is not simply to suppress individual symptoms.
